APPLICATION FOR MALAYSIAN PASSPORT

A. DOCUMENTS REQUIREMENT

i. Two (2) recent photographs see Photographs Specification

ii. MyKad - original and 1 copy in both sides
iii. Hong Kong ID / Macao ID / Permanent Status ID/ Work Permits
- Original and 1 copy each.

B. ADDITIONAL REQUIREMENTS FOR APPLICANT BELOW 18
YEARS OF AGE

v.. Birth certificate - original and 1 copy
vi. Form W (Born Overseas) - original and 1 copy
vii. Mykad - Original and 1 copy in both sides
viii. Either parent's Mykad - Original and 1 vopy

C. ADDITIONAL REQUIREMENTS FOR NEW BORN BABY

ix. Father's Mykad - Original and 1 copy in both sides
x. Father's Malaysian Passport - Original and 1 copy
xi. Mother's Mykad - Original and 1 copy in both sides (If applicable)
xii. Mother's Passport - Original and 1 copy
xiii. Both Parents' Hong Kong ID / Macau ID - Original and 1 Copy.
xiv. Parent's Marriage Certificate - Original and 1 copy.

D. PASSPORT FEE

ADULT
1) HKD 750.00

CHILDREN BELOW 12 YEARS OLD AND ADULT 60 UEARS AND ABOVE
2) HKD 375.00

1. Applicant should presence personally to submit his / her application

2. Submission Time : Mondays to Thursdays 9.00 am - 12.30 pm
Fridays 9.00 am - 12.00 pm

3. Collection Time : On the same day
Mondays to Fridays 4.00 pm - 5.00 pm
